KEYHOLDER LEASE AGREEMENT
Keyholder Agreement - If you will be getting a VPAR key you must fill out this form, and please remember to have your Broker sign it. This form is also needed when transferring to another office.

LOST OR STOLEN ENTRY CARD AFFiDAVIT
Lost/Stolen card Affidavit - If your key is lost or stolen you will need to complete this form in front of a notary. The fee for a replacement key is $200.
